Si ALLERGIE : Dalacine 900 mg + Genta 5 mg/kg
- Préoxygénation Proclive 25° - VSAI=4 cmH2O PEP=7 cmH2O
- Prévention hypothermie

| Induction : |
- Remifentanyl Bolus 1 µg/kg
- Propofol 2 à 3 mg/kg
- Rocuronium 0,6 mg/kg sans dépasser 100 mg
ou Atracurium 0,6 mg/kg
- Lidocaine Bolus 1 mg/kg
- Prévention NVPO : Dexaméthasone 8 mg + Ondansetron 4 mg

| Entretien : |
- Infiltration des trous de trocarts ROPIVACAINE en début d'intervention
- Remifentanyl SAP 0,05 µg/kg/min
- Desflurane BIS entre 40 & 60
- Lidocaine SAP 2 mg/kg/h
- +/- Clonidine 150 µg dans 500ml RL
- Remplissage per-op # 1000ml RL

| Analgésie : |
- Après le test au bleu
- ARRET Lidocaine SAP
- Morphine 0,1 mg/kg IVD puis 10mg S/C /8h si EVA > 4
- Perfalgan 1 gr puis 1 g /6h
- Profenid 100 mg puis 100 mg /8h pendant 24h
- Tramadol 100 mg puis 100 mg /8h

- Au début de la fermeture
- ARRET Remifentanyl SAP et Halogénés
- Antagonisation Prostigmine 40 µg/kg ou Sugammadex 200 mg IVD SYSTEMATIQUE
